We provide regulated adult social care to adults of working age and people aged 65+. We deliver visiting home care services across Tonbridge, Tunbridge Wells and Sevenoaks, as well as live-in care across South East England.

Working together
We work with a range of local and regional organisations to deliver community social care services. These organisations include GP practices, hospitals, district nursing services and specialist healthcare teams. We also work in partnership with our clients, their loved ones and voluntary sector groups to make sure every individual has a positive experience.
Nationally, Health and Care Partnerships (HCPs) bring together all providers of health services in a given area to work as one. Partnerships can design and deliver services to meet the needs of everyone they serve based on their local population. HCPs are supporting healthcare providers to work more closely with local councils – not just with social care and public health colleagues, but also education, planning, housing, environmental health and leisure services.
In West Kent, the HCP consists of:
- NHS providers: MTW, KCHFT, KMPT and NELFT
- West Kent Primary Care and the primary care networks
- Kent County Council
- West Kent district councils: Maidstone, Sevenoaks, Tonbridge & Malling and Tunbridge Wells
- Voluntary and community sector partners: Healthwatch Kent, Involve Kent
- NHS Kent and Medway Integrated Care Board
