Live-in Care Practitioner

Overview

We’re looking for an experienced, compassionate live-in care worker for a couple in Tonbridge. This placement involves supporting a gentleman with diabetes who is a full-time wheelchair user and requires assistance with the use of a Sara Stedy. The gentleman lives with his wife who has light support with personal care (mild incontinence).

Double-up support is provided daily in the morning and evening. During the day, meal prepration is required incl. breakfast, lunch and dinner, as well as organising appointments, catheter care, assistance with toileting and light housekeeping.

⏳ Includes: 2-hour daily break (2pm to 4pm).

🔹 Requirements:

  • Transport: Must be a driver or comfortable with public transport
  • DBS: Either on update service or new DBS can be arranged (free)
  • Communication: Clear English speaker

Weekly Rate: £900/week
Meal Allowance: £40/week

Responsible For
Service user safety
  • Developing practice to meet core skills, and working within competency framework relevant to the area
  • Maintaining at all times a kind, caring and professional attitude towards service users, relatives and colleagues, taking into consideration dignity, privacy, and confidentiality, cultural and religious needs
  • Safeguarding people by recognising and responding when an adult or child might be at risk from abuse but also recognising their own limits and asking for help and escalating concerns when necessary

Clinical outcomes
  • Supporting service users to maintain their independence and confidence by taking an enabling approach to providing person-centred care and support
  • Encourage and promote service user involvement in planning and delivery of care, maximising independence
  • Work with, and in support of, all other colleagues and stakeholders involved in the care of service users, including allied health professionals, nursing colleagues, and third-party providers, commissioners, and support networks
  • Assisting service users with all aspects of personal care and hygiene, promoting dignity, respect, and privacy at all times
  • Support service users with meal preparation and monitoring of flood and fluid intake
  • Assistance with medication management, maintaining appropriate record sand ensuring the highest standards of medication safety
  • Undertake moving and handling operations in-line with manual handling legislation
  • Working with other team members to ensure that plans of care are carried out to the agreed standard
  • Sharing good practice in order to improve standards of care
  • Acting as service user advocates in the multidisciplinary team
  • Identifying emergency situations and taking appropriate actions
  • Being responsible for own delegated tasks.
Service user experience
  • Working in partnership with service users and their families and carers
  • Gaining consent and, as far as possible, involving people in all decision-making
  • Signposting service users and carers to registered practitioners to further support their care
  • Reassuring people by being professional, responsive, knowledgeable, and confident
  • Escalating concerns and care outcomes to seniors.
Supporting yourself and others
  • Participating constructively in own supervision and annual appraisal processes
  • Being responsible for own personal development, including keeping up to date with statutory and mandatory training and training relevant to any specialised practice
  • Identifying own development needs and taking action to enhance own knowledge and skills and areas of competence, considering organisational needs and objectives
  • Reviewing and reflecting on own competencies and performance through effective use of operational supervision and appraisal and identifying any personal training needs or skills gaps.

Additional Requirements
  • A full and valid UK driving licence is preferred but is not essential.
Educational Requirements
Essential
  • Functional Skills at Level 2 (Literacy and Numeracy / GCSE grade A – C or equivalent in English and Maths)
Desirable
  • Relevant Level 2 qualification NVQ/QCF certificate of care / apprenticeship or equivalent – or willingness to obtain
